Before & After Content on Instagram: Stay Compliant, Still Convert

Before/after is the most persuasive content a clinic can publish. It's also the fastest way to get your account shadowbanned or suspended if you do it wrong.
What Instagram actually bans
Instagram's community guidelines restrict:
- Graphic medical imagery (blood, needles inserting into skin, raw post-procedure swelling).
- "Shock" framing that exaggerates the negative side of the "before".
- Quantitative weight-loss claims and unrealistic transformation promises.
What's allowed: tasteful side-by-side stills with educational framing.
The 5-rule compliance framework
- Same lighting, same angle, same expression in before and after.
- No raw post-procedure shots. Wait for swelling to settle (usually 2 weeks).
- No needles, blood, or active injection footage in feed posts.
- Educational caption explaining the treatment, downtime and realistic results.
- Watermark with consent confirmation ("Shared with patient consent").
Why this still converts
Compliant before/afters perform better than shock content because:
- They feel trustworthy, not predatory.
- High-paying clients want a clinic that respects privacy.
- The algorithm rewards posts that don't get reported.

A 7-second carousel reel: still before, still after, 3 lines of text overlay explaining the treatment, calm music, founder voiceover in the caption. Pin it. Repurpose monthly.
